A Wellness Incentive Program

The HealthePERKS program offers financial incentives through the HealthePERKS Reward Card. The card provides motivation to change unhealthy behaviors such as smoking, not exercising, and being overweight.

As the participant accumulates points under the program, they redeem points for credit on their card. This gives the participant an ongoing reward for consistant behavior to improve their health over the long term.

Rewards are given for things such as:

Annual physicals
Quitting smoking
Losing weight
Exercising regularly

To receive their points, patticpants fill out a submission form and sign an acknowledgment statement verifying their claim. This form is then sent to the program administration. Points are paid out to the program participants on a quartly basis.

The Health e PERKS card is issued by West Suburban Bank pursuant to a license from Visa U.S.A. Inc
